Support Options for Qualifying Low-Income or Disabled Customers

Ozark Fiber is actively participating in the following federal and Missouri support programs. Please follow the steps below to get additional information to enroll or call us for help.

  • Affordable Connectivity Program (ACP) – federal assistance of $30 per month towards internet service for eligible households.
  • Lifeline – federal assistance of $9.25 per month towards internet service for eligible customers OR federal assistance of $5.25 per month for phone service, plus state of Missouri assistance up to an additional $18.75 per month for phone service.
  • Missouri Disabled Program through the Missouri Universal Service Fund – state of Missouri assistance of up to $24.00 per month for phone service. If you need more information, please contact us.

Affordable Connectivity Program (ACP) Information

The ACP is managed by the Federal Communications Commission (FCC) in collaboration with the Universal Service Administrative Company (USAC). The FCC program is designed to assist families and households who are struggling to afford internet service. The ACP is non-transferable and limited to one monthly internet discount per household.

Step 1: Check your ACP eligibility requirement with USAC by clicking here.

Step 2Register through USAC’s ACP National Verifier website by clicking here.  This tool has been created by the US Government specifically for this purpose.

Step 3: Once approved by USAC and to request the $30 per month credit from Ozark Fiber, complete and submit the form on this page.  Please ensure that you use the exact information provided to the National Verifier website to allow Ozark Fiber to accurately confirm your approval.

Step 4: Once confirmed by Ozark Fiber, we will email you an ACP “Opt-In” form, which must be completed, signed and returned before credit will be applied to your account.

Lifeline Information

Lifeline is also administered by the Universal Service Administrative Company (USAC). The sign-up process is separate, yet similar, to the ACP sign-up process.

Step 1: Check your Lifeline eligibility requirement with USAC by clicking here.

Step 2: Register through USAC’s Lifeline National Verifier website by clicking here. This tool has been created by the US Government specifically for this purpose.

Step 3: Once approved by USAC and to request the credit from Ozark Fiber, complete and submit the form on this page. Please ensure that you use the exact information provided to the National Verifier website to allow Ozark Fiber to accurately confirm your approval.

Step 4: Once confirmed by Ozark Fiber, we will email you a Lifeline “Opt-In” form, which must be completed, signed and returned before credit will be applied to your account.
